Special enquiry launched to establish what happens to discharged patients

Healthwatch England are running a special enquiry called ‘Then what…..?’, to find out what happens when people are discharged from a hospital, care home, or secure mental health setting. The local Healthwatches for BANES, Bristol, Somerset and South Gloucestershire want to find out about your experiences of the discharge process.

Have you or your family or friends had any good or bad, experiences that we can tell Healthwatch England about? All of your personal information will be kept anonymous. Your stories will be used to tell the Secretary of State for Health and other key decision makers about real people’s experiences of discharge, and to make recommendations for improvements.
